TNFα Therapy in Crohn's Disease: The Magic Bullet?

Dr. Gary Wild, an associate professor of medicine in the Division of Gastroenterology at the McGill University Health Centre in Montreal, and CIHR Visiting Scientist at the Whitehead Institute/M.I.T. Centre for Genome Research, explores the role of TNFα therapy in the treatment of Crohn's disease (CD). He discusses the goals of therapy for CD and the pathogenesis of the disease, examines the role of TNFα and TNF receptors in CD, and reviews the evidence illustrating the effectiveness of TNFα therapy for the treatment of patients with active CD. Dr. Wild includes a comparison of the two anti-TNF agents, infliximab and etanercept. He then focuses specifically on the results from the ACCENT I and ACCENT II trials, and discusses the clinical, genetic and biological predictors of response to infliximab.
